1. Pick up a form at the Welcome Desk, fill it out and place it in one of the offering boxes,

2. Send non-confidential requests to Prayer Request , or

3. Use the form below.

4. If you have a confidential prayer request, please call 408.359.2112, ext. 110. These requests may be seen by the prayer team and/or pastors. We cannot guarantee privacy for requests submitted via the website or email.
