Dear Parents,

We’re excited that you have chosen UCUMC’s Vacation Bible School for your child!

To register your child we require a complete packet of these three (3) items for EACH child:

1. Child Registration Form
2. NOTARIZED Medical Release Form
3. $20 registration fee (checks made out to University Carillon United Methodist Church)

Registration for your child for VBS will be final when the entire packet is filled out and turned NO LATER THAN SUNDAY, JUNE 10th. No child will be registered without this COMPLETED packet. We will only email you if there is a missing piece or a problem with registration, but a spot will not be reserved until the issue is fully addressed. If you do not receive an email, assume that your child is registered.

One week before VBS, an email will be sent with specific information to all parents of registered children. Check your junk mail!

* Children and youth volunteers must also fill out and notarize the UCUMC consent medical release form to be registered for VBS.
